The mornings are a little darker, the evening walk gets skipped, and the second helping of pasta feels earned. By late October the pattern has a shape: sleeping nine hours and still tired, cancelling plans, a flatness that doesn&#8217;t lift. If that description matches your last two or three autumns, you may be dealing with seasonal affective disorder, and the most useful thing to know about it is this: the calendar is predictable, which means you don&#8217;t have to wait for it to arrive.<\/p>\n\n\n\n\n<h2 class=\"wp-block-heading\">What Seasonal Affective Disorder Actually Is<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The phrase &#8220;winter blues&#8221; undersells it. The <a href=\"https:\/\/www.nimh.nih.gov\/health\/publications\/seasonal-affective-disorder\">National Institute of Mental Health<\/a> classifies SAD as a form of major depression with a recurrent seasonal pattern, with symptoms that typically run four to five months of the year. Alongside the usual markers of depression \u2014 low mood most of the day, loss of interest, trouble concentrating, feelings of worthlessness \u2014 the winter version has a distinctive signature: oversleeping, overeating with a specific pull toward carbohydrates, weight gain, and a social withdrawal that people often describe as wanting to hibernate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two details in the diagnostic criteria matter. The episodes have to occur in the same season for at least two consecutive years, and seasonal episodes have to outnumber the non-seasonal ones. NIMH also separates SAD from the &#8220;holiday blues&#8221;: SAD tracks daylight hours, not the calendar.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A <a href=\"https:\/\/www.cochrane.org\/evidence\/CD011269_light-therapy-prevention-winter-depression\">Cochrane review<\/a> puts prevalence between 1.5% and 9% depending on latitude \u2014 the farther from the equator, the more people affected. It is more common in women and in people who already have depression or bipolar disorder. (There is a rarer summer version too, which we covered in our piece on <a href=\"https:\/\/myamanapp.com\/blog\/summer-depression-heat-mental-health\/\">summer depression and extreme heat<\/a>.)<\/p>\n\n\n\n\n<h2 class=\"wp-block-heading\">Why September Is the Right Time to Plan<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Here&#8217;s what makes SAD unusual among mood disorders: you can see it coming. NIMH says so directly \u2014 because onset is so predictable, people with a history of the disorder may benefit from starting treatment before the fall, and a clinician can help decide both what to start and when.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A caveat worth stating plainly: the evidence for <em>preventing<\/em> SAD is thin. The Cochrane team searched nearly 4,000 records for trials of preventive light therapy and found exactly one, with 46 participants and serious methodological problems. Their conclusion was that no firm claim can be made either way, and that the choice should rest heavily on the patient&#8217;s own preferences. So the case for planning in September is not &#8220;the research proves prevention works.&#8221; It&#8217;s simpler: the treatments that work once symptoms appear all take time to set up \u2014 a light box to buy, a therapist to find, a medication to reach full effect \u2014 and someone who is already depressed is the person least likely to do that legwork. Doing it now, while you still have the energy, is the point.<\/p>\n\n\n\n\n<h2 class=\"wp-block-heading\">Light Therapy: What the Research Supports<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Since the 1980s, bright light has been the standard first-line treatment for winter-pattern SAD. The protocol, per <a href=\"https:\/\/www.nimh.nih.gov\/health\/publications\/seasonal-affective-disorder\">NIMH<\/a>, is a 10,000-lux light box \u2014 roughly 20 times brighter than ordinary indoor lighting, with UV filtered out \u2014 used for 30 to 45 minutes each morning from fall through spring. <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/seasonal-affective-disorder\/in-depth\/seasonal-affective-disorder-treatment\/art-20048298\">Mayo Clinic&#8217;s guidance<\/a> on choosing a box adds the practical details: use it within the first hour of waking, sit about 16 to 24 inches away, keep your eyes open but don&#8217;t stare into it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two cautions. Light boxes are not FDA-regulated for SAD, so make sure the device is built for SAD rather than for skin conditions, which use UV light and can damage the eyes. And if you have bipolar disorder, glaucoma, cataracts, or diabetic eye damage, talk to a clinician first \u2014 in bipolar disorder especially, ramping up light exposure too fast can trigger manic symptoms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The bigger limitation of light therapy isn&#8217;t safety. It&#8217;s that it only works while you&#8217;re doing it.<\/p>\n\n\n\n\n<h2 class=\"wp-block-heading\">The Trial That Compared CBT With Light Boxes<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Kelly Rohan, a psychologist at the University of Vermont, developed a version of cognitive behavioral therapy built specifically for SAD. CBT-SAD runs as two 90-minute group sessions a week for six weeks. It teaches people to notice and challenge the thoughts that winter provokes (&#8220;nothing is worth doing until spring&#8221;) and, through behavioral activation, to deliberately schedule the activities that depression talks them out of.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In a randomized trial of 177 adults published in the <em>American Journal of Psychiatry<\/em>, Rohan&#8217;s team found that six weeks of CBT-SAD and six weeks of daily light therapy produced <a href=\"https:\/\/psychiatryonline.org\/doi\/10.1176\/appi.ajp.2015.15060773\">comparable improvement<\/a> during the treatment itself. The difference appeared later. One winter on, the two groups looked the same. Two winters on, 27.3% of the CBT-SAD group had a recurrence of depression, compared with 45.6% of the light therapy group, and the CBT group&#8217;s symptoms were milder on both clinician-rated and self-report measures.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The explanation is unglamorous but convincing. Light therapy suppresses symptoms only while it&#8217;s actively applied, and most people don&#8217;t restart it the following autumn \u2014 only about a third of the light therapy participants were still using their boxes at follow-up, even though the researchers prompted them and offered to lend the equipment. CBT skills, once learned, come back with you. Rohan was careful to tell <a href=\"https:\/\/psychiatryonline.org\/doi\/10.1176\/appi.pn.2015.12a23\"><em>Psychiatric News<\/em><\/a> that this doesn&#8217;t make CBT the preferred treatment for everyone; the two approaches work through different mechanisms. But for someone whose goal is fewer bad winters rather than one better one, the durability finding is hard to ignore. SSRIs are also used and typically take four to eight weeks to reach full effect \u2014 another argument for starting the conversation early rather than in December.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Vitamin D is the one people most want to work and the one with the shakiest evidence. Many people with winter SAD do have low vitamin D, and NIMH&#8217;s summary is candid: some studies have found supplements as effective as light therapy, others have found no effect at all. Check your levels with a doctor rather than guessing, and mention other medications, since vitamin D can interact with some of them.<\/p>\n\n\n\n\n<h2 class=\"wp-block-heading\">A Practical September Checklist<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If you&#8217;ve had two or more winters that fit the pattern, a few actions this month put you ahead of the curve. Book an appointment with a doctor or therapist now, before wait lists fill in November, and give them your history plainly: which months, which symptoms, how many years. If a light box helped before, pull it out on the first grey morning rather than the first bad week. If therapy is an option, ask specifically about CBT adapted for SAD \u2014 group programs often start in early autumn for exactly this reason. And decide now what your early-warning signs are (sleeping past the alarm three days running, cancelling two plans in a row) so you notice them instead of explaining them away.
