The backpack is by the door, the cereal is going soft in the bowl, and your child is on the bathroom floor saying their stomach hurts. And they mean it &mdash; you can see it in their face. By ten o&#8217;clock, with the bus long gone, the stomachache has quietly lifted. Tomorrow it will be back.<\/p>\n<p>If that is your house this August, you may be dealing with what clinicians call <strong>school refusal<\/strong>. How a family responds in the first two or three weeks tends to matter more than almost anything that happens after.<\/p>\n<p>Parents who have lived this know the particular guilt of it: the suspicion that you are being managed sits right next to the suspicion that something is genuinely wrong. Both instincts are partly correct. That is what makes 7:10 so hard, with work starting in forty minutes.<\/p>\n<h2>School refusal is not the same thing as skipping school<\/h2>\n<p>School refusal is not a diagnosis in the <em>DSM-5<\/em>. It is a behavior &mdash; persistent, anxiety-driven avoidance of school &mdash; that rides on top of something else: separation anxiety, social anxiety, a specific phobia, depression, or an adjustment reaction to something that changed. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK534195\/\" target=\"_blank\" rel=\"noopener\">Clinical reviews<\/a> put it at roughly 2 to 5 percent of school-age children, with two clusters: ages 5 to 6, and ages 10 to 11. Those are the seams &mdash; the years kids start school and the years they change schools.<\/p>\n<p>Truancy looks different from the outside. A truant child hides the absence, isn&#8217;t home during school hours, and is usually somewhere more interesting. A child with school refusal does the opposite: the parents know exactly where they are, the child is home, and the child is lobbying hard to stay there. They are not chasing something better. They are escaping something that feels unbearable.<\/p>\n<p>Function matters more than label. Studying 143 young people with school refusal behavior, Christopher Kearney and Anne Marie Albano <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/14710711\/\" target=\"_blank\" rel=\"noopener\">found<\/a> the attached diagnoses were strikingly varied, and that different underlying motives clustered with different conditions. Two children refusing school in the same hallway can need almost opposite responses.<\/p>\n<h2>Why it snowballs so quickly<\/h2>\n<p>Every morning at home delivers an immediate dose of relief. The stomachache eases. The dread lifts. The nervous system files that away as useful: <em>avoiding school made the bad feeling stop.<\/em> Repeat it a few times and the brain has learned that school is dangerous and home is safe. Rachel Busman, a clinical psychologist at the Child Mind Institute, puts it plainly &mdash; <a href=\"https:\/\/childmind.org\/article\/when-kids-refuse-to-go-to-school\/\" target=\"_blank\" rel=\"noopener\">being absent is very reinforcing<\/a>, and the longer a child misses, the harder the return.<\/p>\n<p>This is also a hard era in which to notice the problem. Nat Malkus&#8217;s <a href=\"https:\/\/www.aei.org\/research-products\/report\/from-attendance-crisis-to-chronic-condition-tracking-post-pandemic-chronic-absenteeism-into-2025\/\" target=\"_blank\" rel=\"noopener\">2026 analysis<\/a> of data from 45 states found chronic absenteeism peaked at 28.3 percent in 2022 and has only crawled down to 22.6 percent, against a pre-pandemic baseline near 15 percent. Most of that has nothing to do with anxiety &mdash; but it means a child missing a lot of school is no longer conspicuous. The anxious kid slips through the crowd.<\/p>\n<p>The physical symptoms complicate things further, because they are real. In 2022, <a href=\"https:\/\/www.cdc.gov\/nchs\/products\/databriefs\/db498.htm\" target=\"_blank\" rel=\"noopener\">5.8 percent of US children ages 5 to 17<\/a> missed 15 or more school days for health reasons, rising to 14.8 percent among children whose difficulties included daily anxiety or depression. The stomachache isn&#8217;t invented. It just isn&#8217;t the whole story.<\/p>\n<h2>The accommodation trap<\/h2>\n<p>Family accommodation is the collection of small, loving adjustments families make to reduce a child&#8217;s distress. Writing the note. Letting them stay home just today. Driving them in at 9:30 to skip the crowded hallway. Answering the midday texts. Picking them up when the nurse calls. Every one is a kind act, and every one is a deposit into the avoidance account.<\/p>\n<p>What lifts this above theory is a randomized trial from the Yale Child Study Center. Eli Lebowitz and colleagues <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30851397\/\" target=\"_blank\" rel=\"noopener\">assigned 124 children<\/a> aged 7 to 14 with anxiety disorders either to twelve sessions of individual CBT, with no parent involvement, or to twelve sessions of a parent-only program called SPACE &mdash; Supportive Parenting for Anxious Childhood Emotions &mdash; in which the child never met a therapist at all. Published in the <em>Journal of the American Academy of Child and Adolescent Psychiatry<\/em> in 2020, the trial found the parent-only treatment <strong>noninferior<\/strong> to CBT on every primary and secondary anxiety outcome, whether rated by independent evaluators, parents, or the children themselves. Accommodation fell in both groups, and fell further in the parent group.<\/p>\n<p>Two things follow. If your child flatly refuses to see a therapist &mdash; extremely common here &mdash; you are not stuck. And, more subtly: SPACE is not &#8220;stop helping.&#8221; It pairs a reduction in accommodation with an increase in supportive response, and the halves are inseparable. Acceptance says <em>I know this is genuinely hard and I believe you.<\/em> Confidence says <em>and I know you can handle it.<\/em> Drop the acceptance and you are simply being harsh. Never reduce the accommodation and nothing changes.<\/p>\n<h2>What to do in the first two weeks<\/h2>\n<p><strong>Rule out the medical explanation once, then stop relitigating it.<\/strong> Recurring morning stomachaches deserve a real look from a pediatrician. But once your child has been checked, resist re-examining the question every morning &mdash; that conversation becomes part of the ritual.<\/p>\n<p><strong>Partial attendance beats none.<\/strong> Exposure works in graded steps. Drive past the school. Walk the empty halls on a weekend. One class. Half a day. A full day. Each rung is evidence the child collects about themselves, and it is the same logic behind <a href=\"https:\/\/myamanapp.com\/blog\/types-of-therapy-cbt-dbt-emdr-act\/\">exposure-based approaches used in CBT<\/a>.<\/p>\n<p><strong>Move the conversation out of the morning.<\/strong> Mornings are for logistics. Evenings and car rides are for talking. &#8220;Are you worried about today?&#8221; at 7:10 hands the anxiety a microphone at the worst possible moment.<\/p>\n<p><strong>Name a specific person at school.<\/strong> Not a general heads-up &mdash; an actual human, an aide or counselor or friendly teacher, who meets your child at the door and immediately gives them something to do. Handing a kid a task works better than discussing how they feel.<\/p>\n<p><strong>Praise the specific thing.<\/strong> &#8220;You walked in on your own this morning&#8221; lands. &#8220;Good job today&#8221; doesn&#8217;t.<\/p>\n<h2>When to move faster<\/h2>\n<p>Some situations call for help now rather than a wait-and-see period: the pattern has run for weeks rather than days; your child attends but lives in the nurse&#8217;s office; something specific is identifiable, like bullying or an undiagnosed learning difficulty producing daily humiliation; or symptoms are escalating into panic attacks, refusal to leave the house, collapsing sleep.<\/p>\n<p>And one that is never wait-and-see. If your child talks about hopelessness, self-harm, or not wanting to be here, get support the same day. In the US, the <strong>988 Suicide &amp; Crisis Lifeline<\/strong> is free and available 24\/7 &mdash; <strong>call or text 988<\/strong>, or chat at <a href=\"https:\/\/988lifeline.org\/\" target=\"_blank\" rel=\"noopener\">988lifeline.org<\/a>. It serves young people directly, and the frightened adults trying to help them.<\/p>\n<p>Short of a crisis, a clinician who works with childhood anxiety can sort out what is actually driving the avoidance &mdash; the step that makes everything else effective. It is for them to <em>go<\/em> while not feeling fine, and to discover, in small repeated increments, that they survived it. The comfortable feeling arrives afterward. Waiting for it to arrive first is how weeks turn into months.<\/p>\n<p>The child on the bathroom floor is not fighting you. They are fighting something that feels genuinely dangerous, and you happen to be standing in the doorway. Knowing that doesn&#8217;t make 7:10 easier.
